If you prefer, you may call the center to schedule a consultation, but please complete the medical history and lifestyle questionnaire prior to your initial visit. All calls will be returned within twenty-four hours.
Dr. Perry also provides phone consultations pertaining to bio-identical hormone replacement, physical fitness, and nutrition counseling and supplementation. Fees charged for phone consultations will be applied to a patient's initial clinic visit should the patient require a more thorough evaluation.
Payment for services
The center accepts cash, personal check, debit card, and credit cards. The policy of this center is to collect payment in full at the time service is rendered. If you chose to pay for services at the center, only cash or check are accepted. The center does not accept insurance for payment; however, you will be provided with a properly coded, itemized receipt that includes the appropriate diagnostic and billing codes. Many insurance plans will cover part of the cost of office visits according to their out-of-network provider schedule and may cover some or all of the costs of laboratory testing.
For your convenience, you may pre-pay for your office visit, laboratory testing, supplements, or fitness training by using the "Pay Now" button below. You do not need a PayPal account to use this service. In the text box beneath "Medical Services", feel free to enter the service for which you are paying.
Please be advised that a $25 fee will be charged for appointments that are not canceled prior to twenty-four hours before the scheduled date of service. There will be a $15 charge for returned checks.
